Living Matrix Questionnaire
All new patients are required to complete the Medical Intake Questionnaire one week prior to appointment.
- If you have MyChart, please look for a message 4 weeks prior to your appointments with a link to complete a Medical Intake Questionnaire. Please note the completion of this questionnaire can take 90-120 minutes or more.
- If you do not have MyChart, look for a message from no-reply+support@livingmatrix.com with a link to complete the Medical Intake Questionnaire.
- If you have not received an email to set up your account, please check your junk mail and look for an email from no-reply+support@livingmatrix.com. If you have not received this email, please call 216.445.6900.
Genova Diagnostics Test Kits
Your provider may order test kits for you to complete through Genova Diagnostics.
Test Kit Instructions
For questions regarding test kit instructions.
- Visit the Genova Diagnostics Test Profiles site
- Search for and select the name of the test in question
- Locate the Collection Instructions link to the right under Additional Resources
Drawing Locations
For Genova test kits requiring blood samples, blood draws can be completed at the following locations:
- Main Campus (walk-ins accepted)
- Functional Medicine Q2
- Internal Medicine G10
- Chagrin Falls Family Health Center (walk-ins accepted)

Insurance and Billing

- To locate the CPT code for your test kit in order to determine if insurance will cover, please follow these steps:
- Visit the Genova Diagnostics Test Profiles site
- Search for and select the name of the test in question
- Select the CPT Codes tab under the test kit name. If this tab is not present, please contact Genova Diagnostics directly by calling 800.522.4762
Test Results
For questions regarding your test kit results.
- Visit the Genova Diagnostics Test Profiles site
- Under the Patients menu, select My Lab Test Results
- Sign in or create an account
- Locate test results under My Lab Test Results
Please note: Test results will not be made available to you until 11 days after the results have been released to your provider. If you need to go over your lab results, please contact our office to schedule an appointment with your provider.
